Legal Question in Criminal Law in Indiana

what legal advice do you have for someone stealing your personal information to obtain telephone service....we called the telephone company and they displayed the information....it seems it's an ex-relative. Do I report to police? And how can I make it not happen again with another type of service

Yes, you will file an ID theft report. You should also obtain a credit report to make sure that there aren't other accounts this person opened and continue to monitor your credit. If there are accounts that are not yours, you need to notify the creditor and the credit report agency. The FTC has a nice website on i.d, theft. You should go to it. there are also attorneys who will do these things for you.
