I have power of attorney over my father. I am needing advice on how to remove my father's name from his utility trailer that I am co-owner of to get him under resources for Indiana Medicaid.

You would need more than power of attorney. It would take a deed and/or bill of sale. However, what you are proposing could very well be regarded as an improper transfer of title and disbursement of your father’s assets, without appropriate equal value being paid to him. This could result in a denial of Medicaid benefits. You probably should not do what you are proposing.
