change last name of minor child

my daughter was proven not to be my ex husbands child he has signed over all legal rights to her.her biological father and i plan to get married he wants his daughter to have his last name. we live in the state of indiana we have little money and we wish to do a legal last name change on our own. the lawyer who did my divorce said i could find the form free fill it out and take it to the court house so it would cost little to nothing except for her birth certificate to be changed where do i get the forms to fill out myself

I thought the form was online, too. It is not - the form is for an adult.

You should probably read the statute first (IC 34-28-2-2): http://www.in.gov/legislative/ic/code/title34/ar28/ch2.html

It is not quite as simple as you might think.
