contempt
ex wife is not following child visitation orders by the court. i need proper forms for citation for contempt in order for her to appear before the courts again.
i am current with child support and this will be the third time i have taken her back to court for this situation. i cannot afford to continue to retain a lawyer in this matter and am attemping to submit this myself.

While you may do this pro se, you are still bound by the same rules and standards as an attorney. In other words, this is not small claims court or Court TV.
This often means that a person going into court pro se does not do well in keeping evidence from the other side out or properly presenting evidence to the court that the person would like considered. In short, there is much more ot it than just having the correct form. This is not the same as filing your 1040EZ form with the IRS.
However, if you are so determine, one option you may have is to go to www.in.gov/judiciary/selfservice/. This will have a number of forms including what you seek as well as other information. You should still excercise caution as what you find and use as this cannot be a substitute for a skilled attorney.
At the same time and in all fairness, some who have gone down this road have achieved their goal. Yet, the sucess rate from what I understand has not been overwhelming.
