Legal Question in Family Law in Indiana

I am looking for forms for change of venue between IN cty Lake to Porter - case filed as thought had to get divorced in county we lived in and I have been in Porter for over 3 months. We are self represented in our divorce and have agreed on everything and have no joint assets save one that I am taking over. Porter Cty will grant without final hearing. Lake County you have to appear for pre and final dates. I need to change the venue (Lake to Porter) if possible or dismiss in Lake County. What forms do and I need and where can I find them to file

There might be a form dismissal at www.in.gov.judiciary. I am unaware of any forms for change of venue.
