Legal Question in Family Law in Indiana

I have full custody of my son. My ex-husband is in and out of my son's life. He doesn't make him shower or brush his teeth. He doesn't satisfy my request not to smoke around my son (he is very allergic and comes home with red puffy eyes and a stopped up nose) and many times he drives him around in vehicles with several other people making it impossible for him to wear a seatbelt. He moves frequently, job hops and is $17,000 behind in child support (which is the least of my concerns). I feel he is an unfit parent. What recourse do I have and where should I begin?
