Geographical Visitation Change

I just recently remarried and my daughter and I are moving to Ohio with my new husband. What forms do I need to file for a change in geographical location/visitation and child support reduction? My ex-husband and I have come to an agreement and I want to know if I can handle this without an attorney.

As there is no pre-printed ready to use fill in the blank form,you will have to draft the appropriate pleading or document. This should be a notice and agreed entry of the parties.

The appropriate facts and information must be included along with the signatures of the parties.

This information should include, but not be limited to the previous divorce action, reasons for the move, the new address and new points of contact as well as a clear statement that the father of the childern does not object.
