Legal Question in Family Law in Indiana

my 5 yr. old grand-daughter is coming to my home in 2 days from another state to spend the summer with me in Indiana. Would it be legal if her mother and myself write out a piece of paper giving me permission for a medical emergency decisions and have it notarized at the bank in case I need it?

I do not know if that will work or not. Mom should leave you an insurance card.
