This is not the correct forum for a complete explanation of Indiana law on grandparent's rights as there are aspects of the law that are rather detailed.

I do not know of any forms for establishing grandparent rights. The law was recently changed so any forms that you might find online may not be appropriate.

I suggest that you hire an attorney. These rights are too precious to waste by trying to do something that most people are not qualified to do. Good luck.
