Legal Question in Family Law in Indiana

I have guardianship of a granddaughter who turns 18 in November. The only benefits she receives are healthcare through Hoosier Healthwise till age 19. She desparately wants to legally change her last name when she turns 18. Can she do that without loss of the health benefits or affecting my guardianship ?
