Legal Question in Family Law in Indiana

Co- Guardianship

My son's father gave up all rights 12 years ago and my mom and dad have helped me get him to dr. appt and such but with the new laws in force most are saying that they need to be a guardian or co-guardian to get him treated. What forms are involved in this and what is the technical term that is involved?
