involuntary, termination of parental rights by a biological parent of a child that is to be adopted, when the parent signs a formal written Relinquishment or a Consent to Adoption document,- is there a web page that i can go to to pint out such documents(surrender a child?)

I do not know of any such document that you can obtain online. Did you check the Law Guru Forms section? To be quite frank, you really should contact an attorney to represent you. This particular legal proceeding has a number of landmines and you want to make sure that everything is done correctly.
