involuntary, termination of parental rights by a biological parent of a child that is to be adopted, when the parent signs a formal written Relinquishment or a Consent to Adoption document,- is there a web page that i can go to to pint out such documents(surrender a child?)

I do not know of any such document that you can obtain online. Did you check the Law Guru Forms section? To be quite frank, you really should contact an attorney to represent you. This particular legal proceeding has a number of landmines and you want to make sure that everything is done correctly. If you are the adopting parent, you want to be sure that the biological parent cannot come back at you later. If you are the parent giving up the rights, you want to make sure everything is right so that noone can years down the line seek support from you. Good luck.
