Minor Last Name Change

I have a 12 year old son (dob 9-11-91) who would like to change his last name to my (his mother) last name. He has only seen his father about 10 times in the past 10 years and his father has agreed to the name change. This is very important to my son who has been thinking about it for 2 years and he would like to do it before school starts. Can you please tell me the process and/or an attorney here in Indiana that can help?

A petition for a name change is filed by the parent in the local Circuit Court in the county where you reside and both parents file a consent to the name change, a notice is published in the newspaper and in about 30 to 60 days the Court holds a hearing and grants the name change in these situations. The Court costs are about $225 and you end up with both a piece of paper with the Judge's signature saying the name is changed and paperwork sent to the Board of Health so that the birth certificate can be changed. A copy of the court order must be sent by you to Social Security for them to change the child's name on its records.
