Proof Of Service

I am trying to do my own divorce (laid off, no money) and my Ex has been served twice, but due to other circumstances has not actually received the papers yet. The courthouse has asked me to get a ''Proof of Service'' form and fill it out. I have googled, been to the library and been to the IN.gov website and cannot find it anywhere. The people at the courthouse aren't forthcoming with where I can get the form. Please help! Thank you.

You can find a proof of service that may need to be modified on a Summons dor Dissolution cases, otherwise you may have to hire an attorney to draft one for you.
