Legal Question in Family Law in Indiana

My question regards the voluntary termination of parental rights of a father in the state of Indiana or Kentucky (depending on if it is where the child lives or the father). Which state would this be sought in? Also, how is this done? Does it have to be initiated by the father or can I (the mother) initiate it and he agree to it? Lastly, what are accepted reasons for this to be done?

If the child lives in Indiana, then Indiana has jurisdiction. Father's parental rights can only be terminated by the parents in conjunction with an adoption by mother's husband. If there is no adoption, then you can't do it.
