Legal Question in Family Law in Indiana

I have two questions

My first question is I have never been married. I have never gotten along well with my father and I want to change my first and last name. How much does it cost to do it? What is the legal process I have to go thru to get it down. The second question I have is I was m y fathers caretaker. My father had a severe panic attack and assaulted me. I did not press charges. My father's girlfriend I was friends with first. Since the incident she caused my dad to turn against me. I was wondering if I could sue her for damages as I would like to go to therapy. There is a do it yourself packet fpr name changes at the Indiana Judiciary web site. You will have to pay the filing and legal publication fee, should be a total less than $200. There are not enough facts presented to allow an aswer to your 2d question.
