One of the sections of my divorce decree states: ADDITIONAL INSTRUMENTS: Each of the parties shall, from time to time, at the request of the other,execute,acknowledge and deliver to the other party any and all other further instruments that may be reasonably required to give full force and effect to the provisions of this Agreement.............
Question: If I request from my former wife her total income for 2009 does the above statement force her to acknowledge. Both incomes have changed in past 5 yrs. and pay child support based on income in 2004 which has dramatically been reduced.

No. That clause is for transferring titles, income tax exemptions, etc. You need to file a Motion or Petition to Modify and ask for this information.
