Legal Question in Family Law in Indiana

My son is 3. I have physical custody we share legal. My son has came home repeatedly in diapers and advises his father us still giving him bottles of milk instead of food. My son is potty trained and off a bottle for quite some time now. Today I met him to pick him up and my son was n car seat up buckled in front seat while his father did donuts n a parking lot. Additionally my son advised he also slept in his dad's bed with him and his dad's girlfriend. He has refused numerous times to buckle him n the back. I will do it and pick him up and he's back up front again.
I have asked him to stop the bottles. Stop the diapers and send adequate supply of underpants and pull ups. My son sleeps n his own bed here at home and I asked please put him in his bed there. He doesn't belong in a bed with his gf at all. Ever.
There is more over the last 6 months. If I ask the court for supervised visits until he gets it under control and un he's not being cool but he's hurting our son would a court be sympathetic to ordering supervised visits on thus type of behavior? I want supervised visits.
