Legal Question in Family Law in Indiana

I currently live in kentucky, my daughter lives in Indiana. I assigned temporary custody to a woman who i have been close with for years. She was my foster mother when i was a child, similar to a mother to me. I had a very rough spot about 3 years ago. This woman has 12 acres in the country, and a nice home, so I asked her to take in my daughter until I could get on my feet. For the last year and a half we have been fighting often. Every time I talk about bringing my daughter home she would become angry and not talk to me or answer my calls emails or texts for awhile. It became evident that I will have to go to court to revoke the temp. guardianship and now I don't get to see my daughter at all. She takes my daughter to a shrink, I believe to try to say later it would be detrimental to her psychological health for me to get her. She is brainwashing my daughter telling her lies about me. There is no relationship with my daughter at all anymore. When she does come home I know it will be an uphill battle to restore what we had. I am in college, and working so I will be doing this on my own. I need to know what forms to file, and how to see a judge about this. I feel like this woman doesn't have a leg to stand on seeing as how she is not related to me, my daughter, or anyone in my family. Please help? I miss my daughter so much.

--the father is not in the pic, never has been and doesnt care to now. also, i cant just go get her. we filed thru the court so it has to be revoked. if it was that easy i would have done that as soon as i was on my feet. i expected the woman to be civil because she knew it was temporary when i left my daughter with her. yes it is my fault because i should have known this woman would not want to let her go, but i have no family, raised in a foster home and i had no one else to turn to.
