If an unmarried woman has a baby and gives that baby the father's last name on the birth certificate, is she able to change baby's last name to hers and is there a time limit to do so?

She can file a request for a name change. It is a new civil action, and will require notice to the babys' father. The court will have to decide whether it is in the child's best interest to change the name. Anyone can request a change of name at any time in their life.
