Statute of Limitation

Is there a time limit that a hospital has to bill a patient? Two and a half years AFTER the service, the hospital is just now billing us. Insurance refuses to pay. (We had not rec'd a bill prior to this. Otherwise, it is certain we would have been in collections long ago.) What recourse do we have?
