my father passed away

is there a specific claim form that needs to be filled out in order for us to pay the medical bills

Yes. The person who claims that the estate should pay must file a claim in the probate estate. The bills are paid out of the assets. You are not personally responsible for them. The person files a claim with the bill and when you pay the bill they are required to file a satisfaction of claim form.
