Legal Question in Family Law in Iowa

Background: We live in Iowa, my daughter is now 10. We have no legal documents on the custody of our child. No child support is paid to either parent. We rotate every other year on who claims her on taxes. I have my daughter roughly 60% of the time.

My daughter�s mother is applying for assisted living housing and is asking me to sign a paper telling them that she has �primary care� of our daughter so that she will qualify for this place. If I sign this paper could she use this in the court system that I signed away the primary rights to her? She claims she will not take me to court but I feel like I need to protect myself.

What would you be signing? A document that has no legal force and effect. Somebody's got to hire an attorney and get a custody and support court order signed by a judge in place.
