Birth Cirtificate

My girlfriend is pregant with my child. Can the child have my last name if we are not married? If she gives the child her last name and list me as the father, and we were to marry in the future would I then have to adopt my child? Or can we give the child both last names? Does the hospital have any say in how we decide?

You need do nothing. You are not required to adopt your own child. If you do have your name on the birth certificate you will be presumed to be the lawful father.
