Giving right to make medical decisions about your child for you

I will be gone for a week and my mother is taking care of my two minor children. I do not have a living will. What kind of form should I use to allow my mother to seek medical care/treatment for my child if I am unable to be contacted first?

You need to properly execute a power of attorney.
