Legal Question in Family Law in Iowa

While in a grandmother care does she have to administer prep. medication to her 5 year old grandson. the medication is highly dangers and prescipted by a general doctor without a complete evaluation of the child. The child is in the mother--name removed--primary care with joint custody with my son who is against this medication given to his son. If a grandmother does not give the medication what can happen to her?
