Legal Question in Family Law in Iowa

In Indiana is it state law that noncustodial parent pays a percentage of uninsured medical? Is it suppose to be in the final decree or can the ex just have her lawyer put in an order for the judge to sign so father has to pay a percentage? Don't remember seeing or knowledge of such an order existing.
