Legal Question in Family Law in Iowa

My mom has guardianship of my daughter and my daughter has lived with us since 10/14/2016 and my mom does not want to end the guardianship so she doesn't lose my ex husband's child support. I need to find a form for Iowa that I can file with the court to end the guardianship on my own.

There is no such form. Living with you for ten days is hardly enough to merit a change in guardianship.
