Motion to modify Child custody form

I need to find a form that will allow me to file a motion to modify a current child custody order. The original order was in a divorce. I have researched that I can file it at the Clerk of Courts, but have to locate the form myself. Can you direct me?

Your local "Family (Pro Se) Court should be able to assist you in preparing and filing a Petition or Motion to Modify the Custody Order included in your Divorce Decree.

This message is from one who practices law in Iowa and spends about 24 hours a day in the jurisdiction. The Clerk of Court in this jurisdiction will NOT assist you with pro se forms. I recommend that you hire local counsel if you can-this is not a game for amateurs. If you are slim on resources you may contact the Legal Aid Society or a clinic operated by one of the two law schools in the state. If you insist on filing your own Petition to Modify I suggest Iowa Pleading and Practice for your resource, which can be found at the two law school libraries in the state as well as the state law library in Des Moines and probably Creighton as well.
