Visitation of my daughter

I have been divorced for almost a year. In the original decree my ex husband has custody and I was to have supervised visits due to an alcohol dependency. He allowed me to have unsupervised visits for quite a period of time. He then got angry because I took her to the pool. I have not seen her since then or been allowed to talk with her on the phone. As of today I have 219 days of sobriety. Do I have a chance at modifying the decree to force him to allow visitation? He changed his phone numbers and refuses to answer emails I send regarding the baby. He has also repeatedly accessed my email accounts without my permission. Any advice would be appreciated.
