After a separation, is it legal to remove spouse from insurance before a divorce is filed?

It's the dumbest thing a spouse can do. Spouses are each liable for the other's medical care, treatment and costs. That means if the one gets sick the other can be sued for the costs of that care. So why take the other off of the health insurance? Makes no sense to me, although it may be legal depending on the state and the plan.
