can my employer deny time off request for an MRI that is being requested by my doctor because of problems I have had for over two months?

That would be a matter between you and your employer's human resources department. As you know, Iowa is a right to work state in which you can be fired for any reason, good reason or no reason at all unless you have a contract of employment.
