How to change name on real estate deed -- what form is needed?

What form/s do I need to complete and file in order to change the name on a property from my deceased mother, to myself? The house has been left to me, probate is open, and my attorney has been incommunicado for over two months, and is generally non-responsive.

A change of title should be done, but the bottom line is, you should probably hire a new attorney if you've been trying to contact him/her for two months with no success. The probate needs to be completed.
