Legal Question in Wills and Trusts in Iowa

I have been a caregiver of 15 years for my grandmother in her home,she is 102 yrs of age. she is on the elderly waiver, in Iowa there is the estate recovery, to move would cause me unduehardship..now 2 weeks ago the doctor placed her in a nursing home,cos the med's he started giving her he recommend for her not to return home. Now the social worker says she can't own any property, that i need to put the house in my name. I don't know how to get this done. And does the estate recovery conflict with this?

Yes. There is a 5 year lookback period for medicaid. Doing a transfer might get you a free place to live for a while
