Under Iowa law, employers are legally obligated to fill out a form known as a "First Report of Injury" any time an employee suffers an on-the-job injury,

whether the employer believes that there has been an injury or not. Further, there are Federal obligations for the employer under OSHA for keeping track of workplace accidents.

I would want to know more about the circumstances of the supervisor's refusal to report the injury. There would also be issues of whether the employer received notice of the injury. Notice can either be given formally, or implied by reason of what the employer knew or should have known about the employee's situation.
