Changing My Daughter's Name From Mother's Maiden To My Surname

I am looking for info to change my daughter's name from her mother's maiden name to my surname. Paternity has been established, I'm paying child support and am very involved in her life. I feel it's her birthright and I intend to give it to her prior to the beginning of her starting school.

I'm not certain, however, I would think you could file a simple Petition to the court w/a copy of your paternity verification, requesting that the court order an amended birth certificate issued. Inquire of your district court clerk.
