commonlaw marriage form

how do I get a form for free to have my fiance and me as commonlaw marriage filled out to send in to my insurance to get him on it and have it legalizied

I do not know of any such form, free or otherwise. I don't mean to quibble with the wording of your question, but if the man is actually only your fiance, that seems to imply that you intend to get married in the future; therefore, he is not currently your common law husband. In Kansas, common law marriage requires that you have the ability to marry, you have the present intention to be married, and that you "hold yourselves out" to the public as now being married. I believe an insurance application that lists the man as your husband will be a sufficient "holding out" and the insurance application should be accepted on that basis. Good luck!
