What forms?
my daughter is going on vacation . i will have her four year old. what form does she need to fill out should something happen to her?

We normally prepare a Special Power of Attorney and Guardianship document which allows the grandparent to care for all of the grandchild's needs in the parent's absence. It is a notarized document. It is not a court supervised guardianship proceeding.
