2)How I fill out the actual Complaint Form.

The cover sheet is for the clerks information, and is not a public record. Check the box that seems to most closely describe what you seek to achieve.

As to the complaint, you may need to hire an attorney for the limited propose of helping you draft your Pro Se pleadings.
