If you qualify for citizenship, you need to file form N-400 with supporting documentation.

You first have to apply for permanent residency (green card). The forms to be filed depend on whether you are applying through your employer or family.
After becoming a permanent resident, you have to reside in the US for 5 years before you can apply for citizenship. There are some additional rules involved.
