Legal Question in Immigration Law in Kansas

I got an RFE for my OPT STEM extension requesting for Employment history from last year july 25th 2012 till now, employer details,job description,job title, hours worked per week, salary, pay stubs, w-2 form. I would like to know few answers for these :-

1) I worked with my first employer for 3months unpaid employment and went to India, should i mention that in my employment history as leave or vacation specifying as i went to my Home country for a month?

2) I dint update my second employer's information on SEVIS in the school, should i mention in my employment history about it?

3) I dint fill my I-9 form with my second employer? How does it effect? But was on pay roll for 3months?

4) Does USCIS know about my pay stubs and hours worked information, if i havent updated in I-9 form and in the school SEVIS? But my employer has updated on ADP.

5) How will USCIS know about my employment history to verify? Will they track with School? Or check with my SSN??
