if someone has a living will that states they do not want artificial nutrition withheld, but then end up with end-stage Alzheimer's, losing the ability to eat, and no hunger, are we required to have a surgical tube inserted? she has no chance of recovery, her doctors agree.

These issues can be tricky, but if all of the family is in agreement, you should be able to let her go rather than prolonging the inevitable through life-support. I would start by discussing the matter with the social services folks at the hospital or hospice center, and with the physicians. The problems you run into in these situations is when one family member is insistent that support continue, while another wants to end support. That's usually when it ends up in court.
