You need to look at the local rules if your court has them. A petition needs to be prepared with the information required by KRS chapter 355. If this is agreed, then an agreement dividing property, providing for custody, visitation and support of any minor children etc needs to be prepared and signed and filed. If there is no agreement then a summons needs to go to the sheriff for service on the other party. Depending upon separation periods and waiting periods if any children are involved, then a hearing needs to be scheduled before the appropriate official. Most circuits in Kentucky have Domestic Relations Commissioners who are fact finders for the Circuit Judge. A few circuits have family courts with a separate judge. In some jurisdictions you go before the Circuit Judge. Look at your local and state rules of court. You may be able to get the circuit clerk's office to let you look at a divorce case. They are all different. There is no "form" as such.
