Forms to reopen case and contempt

Hello. I live in Kentucky. I need to file a motion to Reopen a previous case and motion for contempt. My ex-husband was ordered to pay me reimbursement of $440.50, in December of 2003, by $50.00 weekly until payed off. This money was for court costs and attorney fees for divorce. As of today, December 21, 2004, I have not recieved a penny. I cnan not afford to pay a lawyer again, especially since the fees from before have not been payed. So please help me find out how to prepare these forms myself. I live in Hopkins County, In Dawson Springs. The court house is in Madisonville (zip 42431) where the original case was filed. There are no forms. Each Court generally has a "preferred" style of pleading but so do most Attorneys. Your best bet is to go the Circuit Clerk's office and ask to see a case where someone was asking that a court order be enforced. They may be able to help you. I suspect that you could also log into www.lawguru.com. The best thing that you can do is to see if legal aid can help. Also you could go to the main library at Western Kentucky University or Murray and see if they have a legal library. You may be able to find some form books there. Go speak to an attorney or the one that helped you in the beginning. You may be surprised at the assistance that will get.
