I think you are asking for step by step instructions in forming an LLC. That is really beyond the scope of this forum but I will point you in the right direction. Look up Kentucky Revised Statutes (KRS) Chapter 275 here: http://www.lrc.ky.gov/KRS/275-00/CHAPTER.HTM.
