what forms do I need in Kentucky to change my name on my deed

I can't imagine why you need to. Should you sell or transfer the property, your name on the deed would just be "Jane Smith f/k/a Jane Jones." I guess if you really wanted to, you could get an attorney to draft a strawman deed to make a name change (which you might want to do to add your husband to the deed), but it shouldn't be necessary just for a name change. If you're trying to get a new mortgage or something, a copy of your marriage license will suffice.
