Well, I can only assume you are talking about a document used to denote specific gifts to specific people upon ones death. This practice is accepted to varying degrees in different states/jurisdictions. The best way to do this is to create the document as a hard copy and give it a certain name. Then, draft your will and include a clause in the will that you want the specific bequests to occur as described in this document. The document can then be updated as much as you like without having to re-write a will.

The document must pre-date the existence of the will and the will must specifically incorporate the document by reference.

It is not a fool-proof way of passing things along but it does provide considerable flexibility.
