Power of Attorney
Can you create your own Power of Attorney forms, using the available online forms as a guideline? And if so, would it be recognized as legal if all criteria is met and is properly signed, notarized and registered in your county of residence?

Under the conditions you spelled out in the question then, yes, create away and it should be fine. The danger, of course, is that what your own form says may not have the legal impact your want it to have. You may want to consider getting an attorney to spend 15 minutes reviewing it to see if it will do want you want.
