66 yr old person with dimetia in nursing home. 70 yr old sister has power of attorney, the sister is now

very sick, how to transfer power of attorney of the nursing home brother to a different sibling

Only the principal can give a power of attorney. Since the 66 year old has dementia, you must do an interdiction instead. Consult a lawyer.
